Many thanks Mike for the copy. I can throw this up on a hosting site for download if it makes things easier. I've converted a copy to PDF format for that purpose if needed.

That sounds like a great idea, please keep us informed, I did not see this early enough to have one emailed to me, so I'd be interested in downloading the PDF.
