The only fly-fishing I ever do is in my kilt. I find it a lot more comfortable and it keeps my temperature pretty regular.
As far as freezing my toes off, even the best waders won't keep you from hypothermia in a Scottish river in winter.
You just try to stay out of the water as much as possible when it's too cold. Eventually you build up a resistance.
