I have one of those Cold Steel ones and they are great...but I wouldn't consider them a sgian "don't." They can do some damage.
I second the toothbrush idea! They even make ones that house the toothpaste in the handle, that way avoiding the possibilty of...sporran leakage. Or perhaps a little coil of floss in the handle?
Or how about a mirror, to check the combing job?
A teeny pennywhistle?
Medical card and info in case of emergency?

You seem to want to rule out a knife, and don't drink, so I think the USB drive could be put to the most use. You can get a 4 gig for around $20 now-a-days.

If you are a Piper, how about a storage vessel for an extra chanter reed? Or breath mints/chewing gum? Aspirin?
Refillable cigarette lighter?
Salt and Pepper shaker?
Nail clipper and file?
Handi/sani-Wipes (for those Athletic events with portable toilets)?
Not trying to be controversial but -
For the single male...dare I say...prophylactics?
A tobacco Pipe?
Lip balm holder

A metal fork, for when the fast food joints only have the cheapo plastic ones.
Something to hold a Bic lighter for smokers.
A small magnifying glass for olde gents.
A whiteboard marker holder for teachers.
A pillbox for folks that gotta take meds during the day.
A whistle to hail a cab with.
One of them little James Bond type spy cameras.
A spare key holder for olde men who lock their keys in the car.
A small cannister of eye glass cleaner.

Okay the main good ideas (for me anyway) seem to be:
- Pencil case
- USB drive
- spare key
- comb
- chapstick tube holder
- toothbrush
So, to my mind, the next question is how many of these can be combined?
I think a pencil/key/chapstick holder with a USB drive is pretty doable, if you put a lot of thought into it. Hmm, summer project maybe?
I'm glad I made this thread, these are some cool ideas!

I built some time ago a comb, 10gig USB, Vehicle key thing, that rides around in my sock whether I am kilted, or not. The Top pops off and exposes the USB connector. The comb is hinged and is folded teeth into the side of the USB dongle. The same pivot point (a rivet) goes through the head of the vehicle key. Newer vehicles use huge keys! The key is the "blade". I do get comments when I "stab" the ignition switch, and start the motor.

Jamies I love your Sgian comb, did you make it? if not where did you get it?
the idea of a sgian comb and the myth/story about the early Scots not inventing a comb make for an interesting conversation piece.
I am not a weapons kind of guy so this Sgian don't idea is now on my list.

Thank you, I made that out of a scrap piece of a South American hardwood. Since I often help out at my son's school it is the one I wear most. It isn't particularily tradional but it was fun to play around with the belt sander and see what came out. I think the comb was purchased for under $3 and I already had the wood scrap, 2 part epoxy, and button. For those concerned with sweat a simple leather or vinyl sheath could easily be constructed to keep it clean.
