I am a radiologist (department head at a childrens hospital) and wear my kilts to wokr for various occasions and sometimes just for grins, usually on a day when I do not anticipate much if any direct interaction with patients (I do a fair amount of office administration duty). My department love the kilts, as do the patients, parents, and other doctors and nurses, and my various bosses. I typically dress up a bit from my normal work wear (dress pants and a sport shirt) by wearing a tie and vest or jacket with the kilt, so if anything I am dressing better in the kilt than when not kilted. No problems so far, although I am very careful to minimize my contact with kids (my patient population) and I never go commando/regimental at work just on the outside chance that a mishap were to occur or someone was to question the propriety of my attire.
