I've worn a variety of different knives over the years. Usually, if there's one on my lower leg, it's on the inside!
The main reason for this is to keep it from snagging on or banging against things as I walk along. Accessibility isn't a problem: you just train your body to reach there. Bear in mind though, this is at the ankle, not in the top of a stocking...
Speaking for the purpose the sgian serves now though: it's a piece of masculine jewelry. So wearing it on the inside would seem to defeat the purpose (as it would be less visible).
And I'll admit that the sgian as weapon always seemed a bit odd for me: although it could easily be used as such, it doesn't seem well suited to such (given it's size). The concept of it being developed as a skinning knife makes MUCH more sense!
As for "wearing it on the inside means you're looking for a fight"? That's just absurd. Sounds like someone think folks in kilts wander around looking to duel ala Highlander!

A little bit off topic here. Once I put a butter knife in my hose rather by accident, and if anyone noticed, they didn't say anything.

When I drive the car, the Sgian Dubh generally gets in the way of the shifter. That is on this side of the pond. It has not been a problem in the U.K. I either move it to the inside of the leg, or remove it and place it in a door pocket. I did leave it inside the leg once on alighting from the car. Itcame to my attention when I snagged it on the hose of my left leg.

Sounds like bunk to me. However, when I was diving, I would wear my knife on the inside of my leg to keep it from hanging up on the kelp. Oh, wait,...worn inside the leg is a challenge. That explains why the seals in Monterey Bay were always harassing me,...and here I was thinking they wanted the fish I had speared.
