    Similar to Ron's cutoff jeans, there is Thunderwear.

    Ok, ok, it is a sold as a holster too. But it is basically an undergarment pouch that can be positioned most any way you want.

    If you're looking to make the device hidden (and someone already mentioned this) clip the pump on the inside of your kilt. It is true that the back of the clip will show over the waistband of the kilt, though. It is a difficult thing to make it both hidden and easily accessible. I recommend the Nightstalker sporran from SWK. You can fit everything you need in there...and more. Aside from putting it in the sporran, you could also clip it on the back where the D rings go through.
