I'll second the dental drill. That is one naaaaassstty sound.
Spiders creep me out, if they aren't in an observable type environment, otherwise they're interesting. Tarantulas don't bother me so much though.
I'm actually thinking of buying one just for the sake of getting over arachnaphobia. I've known people who have owned them, and they have never had any issues with that. Neat.
Midterms and finals always instill a sense of fear in me, eventhough I have yet to do poorly on such an exam.

Parasites. Thinking about them makes me nauseous. Gross gross gross. The idea of a worm in my intesti--I gotta go throw up now.

Claustrophobic here.
Although Spiders do freak me out if they are ON me, I am mostly content to be near them. I hate when you see one on the ceiling as you are about to fall asleep.
Humans freak me out more than any situations or creatures.

Chickens!
When I was about 3 or so, we visited a family member in the country who had a neighbor with a big, mean ole rooster wandering about. He jumped me and gaffed me pretty good, even drew blood.
So now, here I am, at 6' - 3", 230 lbs, and toting a phobic fear of chickens! I can control the panicky, sweaty, heat-pounding reaction somewhat through purposeful desensitization over the years, but I still don't like 'e.
Sneaky little b@$!@*ds fi you ask me. Always out to get you.

Honestly.... Butterflies.
Everyone laughs at me. I'm kinda percieved as this tough, outdoorsy, rugby playing guy by my friends, but Butterflies just creep me out.
I had a horrible experience in a place called 'The butterfly kingdom' as a kid where loads of them all landed on me at once and I couldn't see where I was going.
I'm fine with moths.

I will not be laughing at you that is for sure. Being OK with moths is a bit of a puzzel though. Do you know why butterflys but not moths?

Well, when it happened I was about 3 and unsupervised, so I freaked out and jumped in the pool to get rid of them.
Being that it was a hothouse dedicated to pretty butterflies, they were mostly of the lovely, colourful variety. And I think I just associate that with the bad experience.
With moths, they are usually pretty drab and not inclined to land anywhere near me. (Too busy flying around the lightbulb.)

When I was 3, my parents took me to the Shrine Circus in Raleigh. They had the "Clown Parade" and one of the greasepainted monstrosities had a 10-foot neck with a HUUUUUUGE head on it. It came down in my face and I screamed ("Mummy, I may have soiled my linens"...)
Never trusted the festive bastards after that.
Yes...I have issues...
