Generally it would be 'Sister so-and-so phd' (or whatever letters were appropriate). Letter head could read 'Dr. so & so' but she would always be addressed verbally as 'Sister'.

When I look at faculty listings at some Catholic universities, there is usually no indication of membership in a religious order (or even ordination), except for the initials of that person's congregation. One has to assume that their preferred form of address is the academic one, since a person is unlikely to be aware of their religious status, unless they happen to be familiar with those initials. So I suppose that depends on the individual.
