One thing I would really change, though, is the sporran. That Rob Roy style sporran is a little too casual and costumey to work well with the rest of it. A plain black day sporran would work better.
As for the waistcoat, I do not care for the unbuttoned look. It just doesn't work with a kilt. Keep it buttoned up and it looks great!
On canes, I really can't offer any suggestions other than to say that for aesthetic reasons I would avoid a traditional short crook-shaped cane unless it's really medically necessary. If you're having pain and want some extra support, maybe consider a straight stick with a decorative knob top and a metal-tipped bottom. It looks much, much more stylish than a crook-shaped cane with a rubber foot.
