Mine were made by a local tee-shirt/embroidery company. I supplied the artwork-- a simple line drawing of what I wanted, and they did the rest. I selected the shirts I wanted (button down collar, short sleeve, 60-40 cotton poly blend) from a range of about 12 colours, and the cost, including the embroidery was under $20 per shirt.

Now there is a one-time set up fee, which basically covers the cost of scanning your artwork and translating that scan into a computer program for the embroidery machines. This fee will vary with the complexity of the design, and the number of colours used in embroidering the design. However, once you've paid the set up fee, it's yours forever and you can go back to the embroiderers and order more shirts and they'll only charge you for the shirt!

If you don't want the shirt embroidered then the embroiderers should be more than happy to sell you just the shirt, although they may want a minimum order-- usually 4-6 shirts.
