I think the key to using the kilt in a costume is make your costume either historical or go as a "Scottish __________" (fill in the blank with whatever you want. But don't use the route of the mass-produced "Scotsman" costume.
Panache's Scottish butterfly collecter is a good example. Also good would be Scottish zombie, Scottish vampire hunter, etc. Or better yet, just be a zombie who happened to be wearing a kilt.
This way, the kilt is not the costume, but is merely a piece of clothing within the costume theme.

You MIGHT be able to make that work with an "electric cigarette". The ones I smoke have a flow vale, so they only produce their "smoke" (wich is really just water vapor) when there's airflow through them. If one was stuck in the end of the chanter, it SHOULD produce a small cloud of vapor when the pipe is playing.
Of course, it would also mute whichever drone it was in...

I'd be willing to sacrifice one drone for the effect of smoke... considering how cold it gets here on Begger's Night my pipes often sound like crap anyways so two-drones might be a blessing.....

How the heck does that holster stay attached to the kilt?

via a long strap, similar to the modern "drop" holster would be my guess...

I had a great idea the other day- there's a person on eBay who makes Star Trek uniforms. She is going to make me one of these:

kilt cut of course!
I'd also love one of these...

but I can't justify the expense.
