And when you say that she's just jealous, you're belittling the issue and not respecting her, especially when someone suggests just throwing jewelry at the issue or buying fancy clothing. It's a futile gesture, like patting her on the head and saying 'there, there'.
I'm not married to her, but this would be my take on it.
good call Dixiecat. This reminded me of a time that I was in the doctor's office and my husband walked in (not kilted), and the two of them promptly ignored me and started talking about "the game," or whatever 
it may have nothing to do with whether or not she likes you kilted, if your kilt takes center stage all of the time it could really be bothersome, and not have anything to do with jealousy.
Now, you guys know I loooove kilts, and I loooove all of you in your kilts, but I have to sympathize with the OP's wife here.
Maybe all she wants is for you to acknowledge this and to talk to her about it... (and wear pants when she goes to the doc! )
Anyway, just my humble 2 cents.
