I have played with this, too...one thing is the knitted portion is so much more comfortable on the legs...the cotton webbing usually available has little give, so the end result of cotton webbing+fabric is not much more comfortable...just my two cents.

To add more options you could crochet a strap and add tassels to end of this strap.

I prefer using worsted weight. I have one of the devices shown on the first post of this thread, but mine has a hand crank. It is designed more for DK or Sport weight yarn. However, I find the sport weight a little thin for my tastes for the cord. Depending on the worsted yarn I have had to add a little additional weight on the cord as it comes out the bottom of the device.
If you want you can use double pointed needles and knit an I-Cord. This is something I have not tried but it appears to give similar results. More info on the I-cord can be found at:

I got one of those cranky machines, but I didn't think the garters looked right, when I finished them.
