A small caveat to the sizing
I've got 5 Stillwater kilts at this point (2 standards and 3 economy kilts). They were all ordered at 38", and both fit me now. However, because of the extra material that goes into a SWK Standard, my Standards are noticeably tighter at a given hole than my Economy kilts are.
However, it's only a matter of one hole.
Other than that, the sizing is pretty close to perfect.

No Stillwaters, but I can tell you the deal with any off the rack kiltmaker - things are gonna vary from batch to batch. Even with custom kiltmakers when ordering from a distance for the first time you may have to adjust from the first effort. Just the way it is. Of coure the best way is to present yourself to the kiltmaker at their shop or at a games if possible and let them do the measuring.
Pretty much the same with any other garment...sizes can roam.

Jerry's website states that the size of the kilts is the SMALLEST waist size, so a 44" will go up to about a 45 1/2". You can move the straps a couple of inches, but I'd recommend actually getting a 42. Don't take out the basting stitches until you're sure. Call Jerry if you have any questions. He's great to deal with.

I usually recommend ordering a size down from anything that SWK or FC offers, for the same reasons listed above. My 36" kilts are a tad loose for my 37" waist, even on the smallest hole!

When I ordered my Stillwater heavyweight, I was up against a deadline. I measured myself, to see if one of the in-stock sizes would fit. I'm [I]almost[I] a 34. When it arrived, it was loose enough that I was nervous. After wearing it a few times, I've discoved that it's actually incredibly comfortable with the waist a little less snug than I'm used to, and I'm no longer worried about it falling down. If I lose a bit of weight, I might have a problem.
Good luck! I hope that you discover that "close enough" is good enough, like I did.
